Atherosclerosis and blood clots
The plaque may rupture and develop a superficial crack.
Blood clots are deposited on this crack and further reduces the channel through which blood has to flow.
If the clot totally fills up the channel of the artery (bottom right), flow is completely cut off and a heart attack occurs
